can microvascular ischemia in the brain related to long term marijuana use be triggered by pain medication?

my husband had dementia MRI showed microvascular ischemia which with further investigation was linked to marijuana use. when he stopped the dementia stopped.now he had his teeth pulled and has been on pain meds and the dementia has returned. dementia happens when he is NOT under the influence of drugs. Also he had a head ache that doctors attribute to a "mini stroke" just prior to the dementia's return. I realize ischemia is causing the symptoms but could pain medicine be triggering the ischemia? I too am a nurse.

1. My guess is that the ischemia is the culprit and not the pain meds.
2. the pain killers aint triggering the dementia for sure or ischemia
