What are some clinics in San Jose, CA that will refer me for a medical marijuana card?

Im trying to find a clinic with MD's that will document my conditions (severe back pain and insomnia) on a medical marijuana referral form so I can apply for a medical marijuana card. Does anyone know of any in the area, or has anyone used a specific clinic in the area?

1. There's a clinic in Riverside, CA that gives recommendations. They're GREAT-non-profit, seminars, support groups, very active. If that's too far away for you, got to www.canorml.org. On the left-hand side there's a physician locator & you can search for your area.
